Enter a checklist or Adoption Agreement, and submit it online for processing! Get your plan document package back WITHIN MINUTES! No subscription fees. You only pay for the plans you want, when you want them... You simply answer the questions on the screen, hit submit, and you're done!

Your document package will be sent back to you in Microsoft^® Word so you can make modifications if needed or Adobe^® Acrobat PDF format. Have your packages sent back to your e-mail address (or anyone else's) or a secure http download site in an executable or zip file.

Here's all you do:

FIRST-TIME USERS

• Print this page by clicking "File" then "Print".
• Verify system requirements before you register.
• Read the helpful information about processing plans using On-Demand Service before you register.
• Register to use the On-Demand processing option (you'll only need to do this once).
• After you register you will receive an e-mail with the login information and be taken back to the Welcome page.

ALREADY REGISTERED?

• Select Relius Documents ASP from our Web site home page and enter your login information.

DOCUMENT PROCESSING

• Select New/Project from the file menu on the left side.
• Select the product (Plan Checklist or Adoption Agreement). Please note that you MUST first sponsor or have approval from the sponsor to complete the corresponding Prototype Plan Adoption Agreement.
• After selecting the desired product, click "OK". You will be taken to the "What You Need To Know About Pay-Per-Plan". Read this page carefully!
• Once you have agreed to the terms, click "I Agree". The data entry portion of the system will be launched.
• Answer the questions on the screen. Use the handy screen commentary and validation program to assist you in answering the questions. . . review your answers carefully and hit submit when you are done!
• Once completed, you will be brought to the credit card information page (we accept Visa, Mastercard, and American Express). After you complete the payment information you will be brought to a confirmation page. We recommend that you print this page, as it is a form of receipt.
• Within minutes, your document package will be returned to you via e-mail or posted to your FTP site in an executable or zip file. You can even request your files to be in Microsoft Word or Adobe Acrobat PDF format!
